Frontline Essential Workers in one of the above categories who are interested in being vaccinated, please CLICK HERE to fill out the state-wide vaccine pre-registration form if you have not already done so.
Business Leaders who employ frontline essential workers, while the single best way to ensure that your employees receive the COVID-19 vaccine is to have them fill out the statewide pre-registration form, in some instances the health department will work with employers in an effort to vaccinate their entire workforce through a provider such as an urgent care location or a local pharmacy. If you are interested, please fill out this survey. The health department will be evaluated interested and eligible employers in the coming weeks.
